ADHD Treatment Q&A

What does adult ADHD actually look like?

Adult ADHD rarely looks like a child bouncing off walls. By the time you are twenty-five or forty, the hyperactivity has usually moved inward. It becomes restlessness, racing thoughts, and the inability to sit through a meeting without your mind drifting to seven other things. Three presentations are recognized under DSM-5 criteria: predominantly inattentive, predominantly hyperactive-impulsive, and combined. Inattentive ADHD is the type most often missed, especially in women and high-functioning adults. Common adult signs include chronic lateness and time blindness, starting many projects and finishing few, emotional dysregulation including rejection sensitivity, and difficulty with money and paperwork.

Why is adult ADHD so often missed or misdiagnosed?

ADHD and anxiety can look almost identical on the surface. Both can produce restlessness, sleep problems, racing thoughts, irritability, and difficulty concentrating. Many adults reach our practice after years of being treated for anxiety or depression with limited success. A short rule of thumb: anxiety-driven inattention is usually fueled by worry, while ADHD-driven inattention happens regardless of mood. Women with ADHD are more likely to present as inattentive, more likely to mask symptoms by overworking, and more likely to be told they are anxious or depressed instead. Late diagnosis after thirty, forty, or fifty is now one of the fastest-growing reasons adults seek evaluation.

How does MindSpa evaluate adult ADHD, including TOVA testing?

Our evaluation goes beyond a self-report checklist. A questionnaire alone cannot reliably distinguish ADHD from anxiety, sleep deprivation, depression, trauma, or thyroid problems. A complete adult ADHD evaluation at MindSpa includes a detailed clinical interview covering current symptoms, daily function, and history since childhood; DSM-5-aligned diagnostic criteria review; validated adult rating scales such as the ASRS; TOVA computerized testing; screening for co-occurring conditions including anxiety, depression, sleep disorders, and substance use; and a medical history and current medication review. Plan on about 60 to 90 minutes of provider time across the initial appointment, plus the TOVA session.

What is the TOVA test?

TOVA stands for Test of Variables of Attention. It is an FDA-cleared, objective, computerized measure of attention and impulse control used in clinical research on ADHD for more than thirty years. During the test, simple visual shapes appear on screen and you press a button when the target appears and withhold the press when it does not. The test measures sustained attention over time, response time, response time variability, errors of omission as a marker of inattention, and errors of commission as a marker of impulsivity. Your results are compared to a large age- and sex-normed sample. TOVA is one component of a full clinical evaluation, not a standalone diagnosis.

What ADHD treatment options are available?

Medication is not the only option, but for most adults with ADHD it is part of the solution. Stimulants are the first-line class and include amphetamine salts such as Adderall in immediate-release and extended-release forms, lisdexamfetamine such as Vyvanse, and methylphenidate formulations such as Ritalin and Concerta. Non-stimulants take longer to work but may be a better fit for adults with certain cardiovascular concerns or substance use disorder. Common non-stimulant options include atomoxetine (Strattera), guanfacine extended-release (Intuniv), viloxazine (Qelbree), and bupropion (Wellbutrin) used off-label. Cognitive Behavioral Therapy adapted for adult ADHD focuses on practical skills including time management, task initiation, organization systems, and emotional regulation. Combined treatment tends to outperform either approach alone.

Can you get ADHD medication online in Florida?

For non-stimulant ADHD medications, prescribing via telehealth is straightforward. For stimulant medications, which are Schedule II controlled substances, current DEA telemedicine flexibilities allow prescribing by telehealth after a complete evaluation. At MindSpa, if a stimulant is clinically appropriate after a full evaluation including TOVA testing, our PMHNP can e-prescribe to your Florida pharmacy under current DEA telemedicine rules. We do not prescribe outside a proper evaluation and there are no questionnaire-only prescriptions.

What does ongoing ADHD care at MindSpa look like?

ADHD is a chronic condition and the goal of treatment is sustained function across the parts of your life that matter to you. After your initial evaluation and prescription, expect a follow-up within two to four weeks to check response, side effects, sleep, appetite, and blood pressure. Once you are stable, follow-ups typically move to every one to three months. Telehealth follow-ups are available across Florida, prescriptions are handled inside the patient relationship, and dose or formulation adjustments are made when life or side effects change. We coordinate with your primary care provider when appropriate.

Does insurance cover ADHD evaluation and treatment?

We are in-network with Aetna, Cigna, Blue Cross Blue Shield, United Healthcare, Oscar, Oxford, and TRICARE. Coverage details, copays, and deductibles vary by plan and our team will verify your benefits before your appointment. Self-pay rates are $200 for the initial evaluation and $150 for follow-ups. The TOVA assessment is included in the diagnostic workup for self-pay patients, not billed as a separate add-on. Most new patients are scheduled within the same week.
